Privacy Statement for www.wchsys.org and affiliated sites. This Privacy Statement covers the use and operation of www.wchsys.org, www.ecusearch.com, www.equippedforlife.com, and homecarepharm.com.  All these sites operate under the jurisdiction of Washington County Health Systems, Inc.

Information Collection and Use

Forms - There are a number of different forms on this site all of which are located on a secure server for your protection (see Security below). These forms are used to directly request a service. Information submitted via these forms will include the users name and a way to contact once the requested service has been performed.   By filling out the required fields of a form and submitting it, you are agreeing to allow Home Care Pharmacy access to the submitted information.  Since the information is sent directly to the appropriate party within Home Care Pharmacy, there is no Opt-Out option.

Tracking - IP addresses are collected for tracking purposes, but are not used to identify customers.  The information gleaned from this is used to examine how www.wchsys.org is viewed and experienced by users and how it can be improved, modified or updated.

Sharing of information

Information collected on this site is not shared with any outside parties and is kept as a record of the transaction.
